Google is finally rolling out the Health Guardian features it announced alongside the Pixel Watch 5. Starting this week, Pixel Watch 3, 4, and 5 owners can set up Blood Pressure Trends, Insulin Resistance Trends, and Sleep Breathing Quality inside the Google Health app. The first trend reports are expected in early October.
The three features are included at no extra cost on those Pixel Watches. Fitbit Air users will get access to blood pressure and insulin resistance trends later this year, although Google Health Premium will be required.
Your watch is looking for patterns
Blood Pressure Trends works in the background rather than asking users to take a manual reading. Google’s WavesFM model analyzes the shape and speed of the pulse under the skin alongside motion data over time. If it spots a persistently higher trend, the app can surface a monthly summary and suggest confirming the result with a regular blood pressure monitor or discussing it with a healthcare professional.

Insulin Resistance Trends takes a similar long-term approach. The feature analyzes several weeks of physiological data to look for changes linked to metabolic health without requiring a blood test. Sleep Breathing Quality focuses on nighttime breathing, showing how much of each night was spent breathing optimally before rolling those results into a longer-term assessment.
It is worth pointing out that none of this is meant to replace proper medical testing. Google itself says the blood pressure, insulin resistance, and breathing-quality features are for general wellness and should not be used to diagnose hypertension, diabetes, or sleep apnea, or to change medication or treatment plans.
Pixel Watch 4 and 5 can also watch for breathing emergencies

Health Guardian also includes Breathing Emergency Detection on Pixel Watch 4 and 5 in select European countries. The watch can monitor for severe, persistent drops in blood oxygen and prompt an emergency call if the wearer becomes unresponsive. Google says the feature is designed for situations such as choking, severe pneumonia, or accidental drug overdose. It has received CE marking in Europe but has not been cleared or evaluated by the FDA.
